delivery services through SCI. All independent contractors
soliciting to perform vendor services should print and
fill out this PDF form so that SCI may qualify your past
service levels. |

| If you are soliciting as an independent
contractor for the first time, the information included
in this form will give us the information we need to set
you up in our system as a vendor. |

| If you are approved as an independent
contract vendor for SCI, you will be contacted by one
of our client courier companies to negotiate rates for
your services. |
