- PAIC’s comprehensive Occupational Accident
Plan is an affordable alternative to Workers’
Compensation for independent contractors from age
18 to 80. Occupational Accident is not a Workers’
Compensation policy.
- Coverage will be provided when the insured person
is injured due to an on-the-job accident, which arises
in the course of the insured person’s regular
occupation while under dispatch, and on duty for the
courier company listed on the insured person’s
Evidence of Coverage.

- DEDUCTIBLE PER OCCURANCE: $250
- Up to $500,000 in benefits available for covered
medical expenses incurred within 2 years of an on-the-job
accident that is sudden, unplanned, unforeseen, and
unintended.

- Pays 70% of the insured person’s average weekly
earnings up to $500 per week for 104 weeks
- Benefit begins seven days after the insured person
is declared disabled by his physician, which must
occur within 3 months after the date of injury.
- Available to age 70.

| CONTINUOUS TOTAL
DISABILITY |

- Coverage is available to age 65. Insured person
is eligible for benefits after being declared disabled
by his physician, which must occur within 6 months
after the date of injury, and must remain continuously
disabled. Benefits begin after the temporary total
disability benefits cease and the claimant applies
for, and receives, a Social Security Disability award
for the covered injury. Payments will equal 70% of
the insured’s average weekly earnings, less
the primary Social Security Disability benefit, *
up to $500 per week.

| SubContracting Concepts,
Inc requires Independent Contractors (I/C’s) to
provide Occupational Accident or workers’ comp coverage,
at their own expense. SubContracting Concepts provides
Occupational Accident Insurance in the event the independent
contractor chooses not to carry his or her own workers’
comp insurance. This Occupational Accident Insurance is
paid for by a deduction taken out of your settlement check
for services. |

| Your responsibilities after
you get injured are to keep appointments, follow all doctors’
instructions on and off the job, maintain good communication
with SCI’s courier client, and to fully cooperate
with all instructions you are given. Your coverage
ceases after completion of your last delivery, unless
you have been dispatched to deliver a substantial distance
outside your normal delivery area. Claims must be
submitted within 7 days of accident injury. |

Weekly Earnings" shall be calculated as
follows:
For those Covered Persons who have received at
least 26 weekly payments from SCI within a 52 week
period prior to the Injury: 75%of the total dollar amount
of the weekly payments for the 26 week period prior
to the Injury divided by 26 = the "Average Weekly
Earnings".
For those Covered Persons who have received less
than 26 weekly payments from SCI within a 52 week
period prior to the Injury: 60% of the total dollar
amount of the weekly payments received during the
26 week period prior to the Injury divided by the number
of weeks in that 26 week period in which the Covered
Person received weekly payments = "Average Weekly
Earnings"
